Case Management

Transitions of Care

• Facility referred
• Self-referred
  • One hour intake session
    • Up to three sessions per week 
    • Number of sessions total dependent on client need

Social Determinants of Health

• 1-hour intake session is completed where the intake questionnaire is completed and goals are set
• Twelve additional monthly 30 minute sessions are scheduled to complete identified goals. 
• Case Manager may make themselves available by email or text for ancillary support
