We provide a 60 minute intake session to determine what services are appropriate to your needs.
Finding care
Education on how to find providers or care centers for client needs.
Accessing care
Education on how to access care so that client can replicate steps.
Understanding care
Education on how to generate questions for visits and how to ask clarifying questions to maximize understanding.
Evaluating and understanding treatment options
Ecucationon how to understand and compare options.
Resource referral
Staff will utilize internal database and internet databases such as FindHelp to locate appropriate resources for client.
Staff will assist client in accessing resources if indicated per policy
Direct Client Advocacy: staff attends appointment for support and to assist with communication.
Assistance understanding specific diagnosis.
Staff will educate on diagnosis, treatment options, and how to follow up with providers on options.
Client can be referred to CM for assistance with complex diagnoses that require major lifestyle changes.
Assistance with Social Determinants of Health
Staff performs one hour intake to assess what SDoH can be addressed to improve outcomes.
Two additional sessions for a total of three sessions to assist and educate on how to manage SDoH deficits.
